The process of applying to live at the Diamond Willow Lodge is simple. Begin by completing the Core Needs Priority Application and Application form and then take the Medical form to your doctor to complete. 

Bring the completed forms with your Notice of Assessment from your most recent tax return to the site manager at the Diamond Willow Lodge, 162 South Centre Street, Magrath, AB T0K 1J0. You can also start the process by phoning Gloria at 403-758-6866 Ext. 1.
